A registered medical patient in Massachusetts pays no state cannabis tax on a medical purchase. A shopper buying adult-use pays the 10.75% state excise and the 6.25% state sales tax, which adds up to 17%, plus a local tax of up to 3% in towns that charge it. Below are the official rates, a calculator and a table of examples.
The rates
As of October 24, 2026. Source: Cannabis Control Commission, "Taxes and Fees".| Tax | Adult-use purchase | Medical purchase |
|---|
| State excise tax | 10.75% | None |
| State sales tax | 6.25% | None |
| Local tax (a town can choose it) | Up to 3% | Exempt |
| All together | 17% to 20% | 0% |
The Commission puts it in three sentences:
"There is no state excise tax on medical marijuana sales." "There is no standard sales tax on medical marijuana sales." "However, medical marijuana sales are exempt from the local tax option."
Try your own number
Type what you spend in a month, before tax. Pick the local tax if your town charges one. You will see the estimated tax on an adult-use purchase, the tax on a medical purchase, and the difference.
The calculator needs JavaScript. Without it, use the table below: it shows the same estimates for five monthly amounts.

What the savings do not include
- The certification visit. The provider who certifies you sets that price. The state does not.
- The yearly renewal visit with your provider, if your certification has expired.
- The $10 replacement fee if you lose your card. The state can waive it for a verified financial hardship.
- Anything about the price of the product. This page is only about tax.
The medical rate needs a card
You get the medical rate by buying as a registered patient or caregiver. That takes a certification from a registered provider, then registration with the Commission, which is free. The five steps are on the patient hub.
AC Peabody is a medical dispensary being renovated at 25 Newbury Street, on Route 1 in Peabody. It will serve registered patients and caregivers. It has no opening date yet.
Short answers
Is a medical marijuana purchase really tax free in Massachusetts?
The Cannabis Control Commission says there is no state excise tax and no standard sales tax on medical marijuana sales, and that medical sales are exempt from the local tax option. The rates are on its "Taxes and Fees" page.
Do I pay the local tax on a medical purchase?
No. The Commission says medical marijuana sales are exempt from the local tax option. That option lets a host town charge up to 3% on adult-use retail sales.
Why is the calculator only an estimate?
It applies each published rate to the shelf price. The Commission page does not say how the taxes stack on one receipt, so your receipt is the exact number.
What do I need to buy as a patient?
A registered patient shows their program registration card and a valid ID at a medical dispensary. A registered caregiver shows their own card. You have to register first, and the state charges nothing to register.
Does the card itself cost money?
The state charges no fee to register or renew. The provider who certifies you sets the price of the yearly certification visit. A replacement card costs $10, and a hardship waiver is available.
Will AC Peabody be a medical dispensary?
Yes. AC Peabody will serve registered patients and caregivers. It is being renovated and has no opening date yet.
